Forms

  • Visitor Application for Visiting Privileges form (CD-100201.1)
  • Visitor Statement of Understanding Attachment (CD-100201.A) (todos los visitantes deben leerlo y firmarlo antes de cualquier visita)
  • Denial/Suspension of Visiting Privileges form (CD-100201.2)
  • Special Visit Request form (CD-100201.3)
  • Visitor Notification of a Proposed Action form (CD-100201.4)
  • Visitor Notification of Hearing form (CD-100202.3)
  • Visitor Hearing Acknowledgement form (CD-100202.4)

  1. Get the Visitor Application for Visiting Privileges (CD-100201.1) - Use the official form and plan to answer every question.
  2. Fill out every required field - The application is screened for completeness. Include key identifiers like your Social Security number and date of birth.
  3. Read and sign the Visitor Statement of Understanding (CD-100201.A) - This signature is required prior to any visit.
  4. Double-check signatures and dates - Missing signatures or dates are a common reason applications get kicked back.
  5. Mail the completed application packet to the facility visiting coordinator - Applications are mailed directly to the visiting coordinator for processing.
  6. Renew every two years - Even after you are approved, you will need to resubmit the CD-100201.1 renewal on the two-year cycle to keep your visiting privileges current.

Note: The visiting department audits every application for original signatures, dates, Social Security numbers, and dates of birth. If anything is missing, you'll have to resubmit a new application from scratch.

Once your paperwork is processed, the facility updates the offender management database with the decision (approved or disapproved), any visitor suspensions, and visiting records as visits happen. You still can't visit until your name appears on the inmate's current approved visitation list.

If you have questions about the visitation process, contact Family Constituent Services by email at CDFamilySrvcs@cd.nm.gov or by phone at (505) 827-8710 or (505) 231-4762. When you reach out, include the offender's full name and NMCD number, plus date of birth and/or Social Security number if available. Mention your relationship to the offender so they can route your question to the right person.
