Got denied? An administrative review lets you flag what went wrong and ask Visiting Services to reconsider. This works best when the denial was based on incorrect information, someone made an error in determining your eligibility, or your situation has changed and you have updated records to prove it. Be clear and direct about what needs correcting, and submit your request quickly—there's a deadline.
Denied? Act fast. Your request for administrative review must be received by Visiting Services within 60 days of the date on your denial letter. That's "received by"—not postmarked. Don't wait until the last minute to mail it. A late arrival means you lose your chance at review.
Complete the CD‑50 visiting application and submit it to Visiting Services by mail, email, or fax. If you’re mailing it, send it to 3723 Fairview Industrial Drive SE STE 200, Salem, OR 97302‑4975.
Fill out the DOC metal detector medical exception form and authorize a medical provider to verify your condition. Then contact the facility to find out how to submit the completed form to Visiting Services.
Have a pacemaker, metal braces, or use a wheelchair? If a medical device or condition means you can't go through a standard walk-through metal detector, Oregon DOC has a medical exception process for visitors. The form covers situations like implanted devices, orthopedic hardware, and mobility equipment—including whether you're able to stand from a wheelchair. You'll need your doctor to verify the information, so build that into your timeline. This isn't something you can handle at the last minute.
