3. Enter your email address - this is required, so use an email you can access easily.

1. Fill in the AIC’s full name - enter it exactly as it appears in DOC records if you can.
2. Add the SID (if known) - the form asks for the SID number, but it’s listed as “if known.”
3. Choose the institution name - this field is required; select the facility where the AIC is housed (Powder River Facility for this situation).
