What to Do If Your Oregon State Penitentiary Visiting Application Is Denied
A denial doesn't have to be the end. Oregon DOC lets you — or the adult in custody — request an administrative review using form CD 1594.
Complete the Request for Administrative Review form (CD 1594) and send it to the Visiting Services Unit. The deadline is strict: your request must arrive within 60 days of the date on your denial letter. Either you (the prospective visitor) or the adult in custody can submit this request.
Time-sensitive: The 60-day clock starts from the date printed on your denial letter. Factor in mailing time - the form needs to arrive within that window, not just be postmarked.
Fill out CD 1594 completely and attach any documents you want considered. Use the explanation section to clearly state why you're requesting a review and include any information relevant to your eligibility.
