What to Do If Your Visiting Application at Oregon State Penitentiary Is Denied

A denied visiting application isn't necessarily the end of the road. Oregon DOC lets you request an administrative review, but you need to use the right form and meet the deadline.

If your visiting application gets denied, you can challenge that decision by requesting an administrative review using form CD 1594. Either the adult in custody (AIC) or the prospective visitor can submit the request. It needs to go to the Visiting Services Unit.

Deadline: Your CD 1594 administrative review request must be received by the Visiting Services Unit within 60 days of the date on the visiting application denial letter.

Fill out CD 1594 completely. Attach all supporting documentation you want considered, because staff will review those materials alongside your request.
