Paper applications go to SCDC's Division of Visitation and Inmate Drug Testing at P.O. Box 212969, Columbia, SC 29221-2969. The form is SCDC Form 19-127, and it has to come from the incarcerated person - only they can provide it. Before mailing, double-check that the form is fully completed. Missing information slows everything down.

Reminder: Keep a copy of what you mail, and make sure the application clearly identifies the inmate (name and ID number).
