How to Apply to Visit Someone at Anamosa State Penitentiary (print, sign, and mail)

Every adult who wants to visit needs to fill out their own visitor application, print it, and sign it. Electronic submissions aren't accepted - you'll need to mail a paper form with an original signature. Bringing children? They don't submit their own applications. Instead, minors must be listed on an application completed and signed by their parent or legal guardian.

Use the official Iowa Department of Corrections visitor application form (available in English and Spanish). Print it out, fill it in neatly, sign it by hand, and mail it in.

How to Fill and Sign

  • Fill out the application completely for each adult visitor
  • Print the application (no electronic submissions)
  • Sign it by hand (a printed, signed form is required)
  • Double-check that your name and other personal details match your ID

Note: Minors must be listed on a visitor application completed and signed by their parent or legal guardian.

Mail your completed, signed application to: Mount Pleasant Correctional Facility Attn: Central Records 1200 E. Washington St. Mount Pleasant, IA 52641 Include a return address on the envelope, and make sure "Attn: Central Records" is on there so it gets routed correctly.

Once your application is reviewed and approved, you'll schedule visits through the Ameelio App. Iowa DOC uses Ameelio for both in-person and video visits, so it's the same system either way. Having trouble with the app? Remember that scheduling happens in Ameelio, but approval decisions come from facility staff.

Note: Ameelio customer support can’t approve or deny visitation requests. Approval decisions are made by facility staff.

Good news: once you're approved, the Iowa Department of Corrections offers both in-person and video visits at no cost to friends and family.
