Become an Approved Visitor and Manage Visits at Bainbridge Substance Abuse Center

Getting approved starts with one step: fill out the Visitation Request Form and submit it to the facility where your loved one is incarcerated. This form kicks off the entire approval process. Until it's submitted, you're not in the system, no matter how much planning you've done on your end.

1. Find the department’s visit request form. This is the official form used to request a scheduled visit.
2. Start with approval. Before anything else, fill out the Visitation Request Form.
3. Complete the Visitation Request Form fully. Take your time and make sure the form is finished before you send it.
4. Submit the completed Visitation Request Form to the facility. Send it to the facility where your loved one is incarcerated so you can become an approved visitor.
5. Request the scheduled visit through the department’s form. Once you are ready to request a visit time, submit your request through the same official visit request form.

Already have a visit scheduled but need to bring someone else along? You can add visitors through the department's online tool. Use the "Add Visitor" link to look up your scheduled visit, then add the new visitor through that same system.

Need to cancel a scheduled visit? Use the "Cancel my visit" link provided by the department. It lets you find your appointment and cancel it directly. This comes in handy when something unexpected happens: a schedule conflict, travel problems, or illness. Canceling through the official link keeps the visit schedule accurate for everyone.
