What to Include on Your Bent County Visitor Application: ID, Forms, and Common Pitfalls to Avoid
Getting approved starts with a clean application packet. Match the right form with the right ID, follow the rules for adults and minors, and you'll avoid the delays and denials that trip up most first-time applicants.
Start by downloading the Visiting Application from Bent County's Visiting Applications page. The form you need is Visitor Application AR 300-01A. Use the current version - older or unofficial copies can slow things down or get rejected outright.
Fill out AR 300-01A completely, sign it, and return it to the facility where the person you want to visit is housed. That last part matters - applications are processed based on the offender's current location, not a central office.
Adult applicants must include a copy of a current, valid government-issued photo ID. Staff use this to verify your identity during the approval process. Make sure the copy is clear and the ID isn't expired.
Common pitfall: The address on your government-issued photo ID must match the address you write on your visiting application. If they don’t match, your application may be denied.
Each application covers one adult only. If two adults want to visit, each person needs to submit their own application.
- ✓ List minor children on the application of the legal guardian or the immediate family member who will accompany the minor on the visit.
- ✓ If a minor will be accompanied by an immediate adult family member who is not the minor’s legal guardian, the parent or legal guardian must provide express written approval using AR Form 300-01F (Authorization for Minor Child Visitation).
Once you're approved and on the active visitor list, the facility runs a background check every year from your initial application date. This is standard for staying on the approved list long-term.
Go a full year without visiting? You'll be placed on inactive status. To get back on the list, you'll need to reapply and clear a new background check before visits can resume.
- Complete Visitor Application AR 300-01A - Fill it out and sign it, then make sure it’s being returned for the facility where the offender is housed.
- Choose how you’ll submit - You can mail the visiting application to: Attention: Private Prison Monitoring Unit, Colorado Department of Corrections Headquarters, 1250 Academy Park Loop, Colorado Springs, CO 80910.
- Or submit by email - Email the application to Doc_PPMU_Visiting@state.co.us.
Applying for a special visit? Use the same submission routes: mail your application to the CDOC Private Prison Monitoring Unit at CDOC Headquarters in Colorado Springs, or email Doc_PPMU_Visiting@state.co.us. Sending it to the right place keeps your paperwork from getting stuck.
