Special Consent
- ✓ HIV testing, diagnosis, or treatment (requires express consent on the ADOC authorization)
- ✓ Sexually transmitted diseases (STD) status and treatment (requires express consent)
- ✓ Psychiatric disorders/mental health treatment (requires express consent)
- ✓ Drug and/or alcohol use treatment (requires express consent)
Expect to pay a fee. State law allows ADOC to charge for producing health records. Before submitting your request, ask what the fee will be - or at least get an estimate - so you're not caught off guard.
- Get the ADOC “Inmate Authorization for Release of Health Records” form - This is the release ADOC uses for health-record requests.
- Fill in the release details carefully - Complete the recipient and record details you’re requesting, and make sure the dates you list match what you need.
- Mark any special-consent categories clearly - If you need HIV, STD, mental health, or drug/alcohol treatment information, those items require express permission on the authorization.
- Sign and date the authorization - The release is valid for one year from the date it’s signed.
- Confirm fees before you submit - ADOC can charge a reasonable fee for producing records, so ask what costs to expect and how payment is handled.
Reminder: If you change your mind, you can revoke the authorization in writing - just know that anything already released based on the signed form won’t be pulled back.
