Documents Minors

  • A current photograph attached to the CR-2152 (the application won’t be processed without a recent photo)
  • Return the completed CR-2152 (with the photo and required documents) for processing within 30 days

If someone other than the custodial parent will be bringing the child, you'll need additional paperwork. The custodial parent or legal guardian must fill out and notarize the Parental Consent/Release for Minor's Visitation form (CR-2152, page 2). This form has to name the specific adult who will escort the child. One detail that trips people up: the notarized consent must explicitly give the facility permission to search the child during the visit. Without that language, the visit may be delayed or denied.

Kids age six and older need updated photos on their visitation record at specific ages: 6, 10, 14, and 18. Keep track of these milestones so the photo on file stays current.

Reminder: CR-2152 requires a current photograph. If the photo isn’t included, the application won’t be processed.

  1. Fill out CR-2152 completely - Use the child’s information and complete the application in full.
  2. Attach a current photo - A recent photograph must be included or the form won’t be processed.
  3. Add the required minor documents - For children under 16, a copy of the birth certificate and the CR-2152 with a recent photo must be on file.
  4. Notarize CR-2152 page 2 when needed - If the child will visit with someone other than the custodial parent, the Parental Consent/Release for Minor’s Visitation form (CR-2152 page 2) must be completed, notarized, and name the authorized escort.
  5. Return the packet for processing within 30 days - Send the completed forms (with the photo and any required notarized consent) to the Associate Warden/Deputy Superintendent in charge of visitation.
  6. Wait for the decision - Applications are typically approved or denied within 30 days of receipt.

After the facility receives the application, expect approval or denial within 30 days. If you're planning a trip, factor in that processing window - don't count on visiting before the paperwork clears.
