Bringing Children to Visit at Bledsoe: Required Forms, Photos, and Notarization

Under TDOC rules, a

If the custodial parent or legal guardian can't come to the visit - or won't - TDOC requires a separate permission form: the Parental Consent/Release for Minor's Visitation Form (CR-2152, page 2). This form designates who's allowed to bring the child in.

Don't skip this: CR-2152 page 2 must be notarized, and it must include consent for the child to be searched. Missing that search permission can stop the visit before it starts.

The child can only visit with the specific person named on that notarized CR-2152 page 2. That escort must be over 18 and already on the inmate's approved visitation list - so double-check that the name on the form matches someone who's actually approved.

Children age six and older need a recent photograph attached to the visitation application - no photocopies. Attach the photo in the lower left-hand corner of the CR-2152 form, or the application won't be processed.

Photo update rules get confusing. One TDOC policy says child photos are updated at ages 10, 12, 14, 16, and 18. A separate TDOC guide lists updates at ages 6, 10, 14, and 18. Because of that mismatch, follow whatever instruction you're given for Bledsoe - and verify the update schedule with visitation staff if you're unsure which ages they're enforcing.

  1. Attach a current photo and submit the application promptly - prospective visitors are expected to return the forms with a current photograph to the Deputy Superintendent/Associate Warden in charge of visitation within 30 days.
  2. Plan for processing time - applications should be approved or denied within 30 days of receipt.
  3. Set a reminder to refresh the paperwork - visitation applications must be updated every two years (24 months).
  1. Decide who will escort the child - if the custodial parent/legal guardian isn’t going, use CR-2152 page 2 to name the authorized escort.
  2. Get CR-2152 page 2 notarized - the consent has to be properly notarized before it will work.
  3. Confirm the escort is eligible - the authorized person must be over 18 and on the inmate’s approved visitation list.
  4. Include the child’s documents - submit a copy of the child’s birth certificate.
  5. Add the child’s photo if they’re 6+ - attach a recent photo (not a photocopy) to the application as required.

Timing tip: Don't wait until the week of your visit to submit paperwork. Applications take about 30 days to process, and forms are expected back within 30 days too.
