The CR-2152 gets you added to an inmate's approved visitor list. Every applicant goes through an NCIC background check, so approval takes time. The person you're visiting can request blank forms from the facility and mail one to you. Once you've filled it out, send it back with a current photograph to the Deputy Superintendent/Associate Warden in charge of visitation.

First, get a blank CR-2152. TDOC provides these to inmates, so the person you're visiting can request one and mail it to you. Once you have it, complete the form, attach a current photograph, and send everything to the Deputy Superintendent/Associate Warden in charge of visitation.

  • Attach a current, original photo (not a photocopy) to the CR-2152.
  • Make sure the application is mailed in an envelope addressed to the Associate Warden of Security (Deputy Superintendent at MLTC).
  • Include the prison name and prison address on the envelope so it routes correctly.

Complete every section of the CR-2152 and answer honestly. Staff use this form to decide whether to approve you, and missing or inaccurate information slows things down - or gets you denied. Don't guess or "clean up" details. Falsifying any part of the application means you'll be disapproved and blocked from resubmitting for at least six months.

Warning: If you falsify any part of the CR-2152, you can’t submit a new visitor application for a minimum of six months.

Once the form is complete, mail the CR-2152 and your original photo to the Associate Warden of Security (or Deputy Superintendent at MLTC). Include the prison name and address on the envelope. Double-check you're using Bledsoe's correct mailing address before sending - wrong addresses mean delays.

After the facility receives your application, expect a decision within 30 days. You typically can't visit until you're approved. One exception: immediate family of newly committed inmates may visit briefly before approval comes through, but that temporary window can't extend beyond 60 days from the inmate's intake date.
