Requesting ADA Testing Accommodations for Louisiana Jailer Exams (what to submit & deadlines)

The Louisiana Office of State Examiner (OSE) provides reasonable testing accommodations under the Americans with Disabilities Act (ADA). These accommodations adjust standard testing conditions so a candidate with a disability can access the exam, without compromising the exam's validity, creating an unfair advantage, or placing an undue burden on OSE. A diagnosed condition alone doesn't automatically mean accommodations are needed. OSE looks for documentation that connects a disability-related limitation to the specific adjustment you're requesting.

Plan on doing two things, not one. You still need to submit a completed examination application. But on top of that, you also need the OSE Testing Accommodations Request Form plus supporting documentation from a qualified physician or specialist who focuses on the disability.

Your supporting documentation needs to be a specific kind of provider letter, not a quick note. OSE requires an original letter dated within the past three years, typed on official letterhead, from a qualified physician or health care provider who specializes in the disability. The letter has to do more than confirm a diagnosis. It needs to document the disability and its severity, describe the limitations you experience because of it, and state exactly what testing accommodations are required. OSE also requires the provider's original signature and credentials on the letter.

Note: OSE will not process your Testing Accommodations Request Form if it is missing your signature. Don’t submit the packet until you’ve signed it.

Already been approved? If OSE granted you ADA testing accommodations within the past three years and you're requesting the same ones again, you still need to complete the ADA Request form. But in that case, OSE generally won't require additional documentation.

Timing matters. OSE requires all documentation described in Section 2 to arrive no later than two weeks before your scheduled test date. If the documentation is insufficient, OSE may deny the accommodation request.

Double-check the signature line before you send anything. OSE will not process accommodation requests without your signature on the Testing Accommodations Request Form. OSE also requires ADA accommodation requests (and any Veteran's points requests) to arrive by the application deadline. Email everything to OSEregionaltesting@la.gov.

Prepare Packet

  • Completed examination application
  • Completed OSE Testing Accommodations Request Form, signed by you
  • Supporting documentation: an original provider letter on official letterhead dated within the past three years, documenting the disability and limitations, stating exactly what accommodations are required, and including the provider’s original signature and credentials
  1. Ask your provider early for the exact letter OSE requires. It must be on official letterhead, dated within the past three years, and include the disability, your limitations, the exact accommodations needed, plus the provider’s original signature and credentials.
  2. Submit Section 2 documentation at least two weeks before your test date. If it is received later than that, OSE may deny the request for insufficient documentation.
