How to Get Approved to Visit Someone at Coffee Creek Correctional Facility
Getting approved to visit at Coffee Creek starts with the Oregon DOC visiting application (CD 50). Once you know where to send it and what might delay approval, the whole process gets a lot easier.
Before you try to schedule an in-person visit at Coffee Creek Correctional Facility, make sure you're on the adult in custody's (AIC's) approved visitors list. The online scheduling system might let you pick a time slot, but the institution can still deny your visit if you aren't approved. Get on the list first - it'll save you a wasted trip.
Note: Submitting a CD 50 isn’t the same as being approved. The AIC also has the right to refuse visiting requests from prospective visitors.
Step1 Cd50
- ✓ Use full legal names (yours and the AIC’s), and include any known aliases when the form asks for them.
- ✓ Add the AIC’s SID if you know it.
- ✓ Clearly state your relationship to the AIC.
- ✓ Answer the criminal history questions honestly and completely.
- ✓ Fill in your current contact information carefully so you can be reached if something needs clarification.
- ✓ Don’t leave required fields blank - missing information can slow review, and submitting the form does not mean you’re approved.
- ✓ Keep expectations realistic: even with a complete application, the AIC can still refuse visits from specific people.
Once your CD 50 is filled out, you can mail it to the Visiting Services Unit. This works well if you want a paper trail for your records.
Fax is another option. Send it to (503) 373-1173 and keep your confirmation page - if there's ever a question about whether it went through, that receipt helps.
Email works too. Send the completed CD 50 to DOC.Visitors@doc.oregon.gov - often the quickest method since you don't have to wait on mail delivery.
Reminder: Sending the CD 50 doesn't guarantee approval. Save your fax confirmation or sent email so you can prove when you submitted it.
Before you invest time in the application, confirm the AIC's current status. At Coffee Creek, adults in custody going through intake aren't eligible for visits. You may need to wait until intake is complete before anything can move forward.
If your application is denied, you can request an administrative review using form CD 1594 (Request for Administrative Review). The deadline is strict: Visiting Services must receive your request within 60 days of the date on the denial letter. Fill it out completely and attach any supporting documents so the reviewer has everything in one packet.
