What to do if your Coffee Creek visiting application is denied

Read the denial letter carefully and note the date on it. Oregon DOC ties your administrative review deadline to that letter date - the Visiting Services Unit must receive your request within 60 days. Wait too long, and you'll miss the window even if you do everything else right.

Keep the denial letter: The 60-day deadline is counted from the date on that letter, so hang onto it and use it when you’re planning your next steps.

What to do if your Coffee Creek visiting application is denied

Got denied? You can ask for a second look. Either you (the prospective visitor) or the adult in custody (AIC) can request reconsideration by completing form CD 1594 - the Administrative Review form - and submitting it to the Visiting Services Unit. This is Oregon DOC's formal process for reviewing denials.

  • ✓ Make sure the Administrative Review request (CD 1594) is received by the Visiting Services Unit within 60 days of the date on the visiting application denial letter.

Once you've completed CD 1594, send it to the Visiting Services Unit by email at DOC.Visitors@doc.oregon.gov or fax it to (503) 373-1173.

Deadline reminder: Your request must be received within 60 days of the denial letter date. Mailing? Use a method that confirms delivery. Faxing or emailing? Save your confirmation.
