If you have a medical implant or device that could trigger a metal detector, you'll want to complete the Oregon DOC Metal Detector Medical Exception Form before visiting Coffee Creek. The facility's visiting checklist specifically mentions this form as something to bring "if applicable." This isn't just a doctor's note you carry with you.
