Bringing Your Kids to Visit: The Authorization Form and Documents You Need for CSP
Bringing kids to visit at Colorado State Penitentiary (CSP) comes down to two things: getting them listed on the Visitor Application and having the right paperwork ready if someone else will bring them.
Any child visiting CSP must be named on the Visitor Application. Good news: minors don't count toward the twelve (12) approved visitors on the offender's list. Adding your kids won't take up an adult visitor spot.
When you submit the visiting application, include a copy of a valid, unexpired government-issued photo ID. Pay attention to the address: the address on your photo ID must match the address on the visiting application, or the application may be denied.
Form 300-01F (Authorization for Minor Child Visitation) applies to one specific situation: when someone other than the child's parent or legal guardian will bring the minor to visit. If you're the parent or legal guardian but won't be the one taking the child in, complete this form ahead of time - otherwise, the visit could get stopped at the door.
- Confirm who will bring the child - if it’s not the parent/legal guardian, you’ll need Form 300-01F.
- Complete Form 300-01F (Authorization for Minor Child Visitation) - the parent must fill it out for the minor who will be visiting.
- Submit the form the way CSP requires - follow the facility-specific visiting instructions for where and how to turn it in.

When listing a minor on your visiting application, CSP requires proof of the relationship or legal authority. This typically means a copy of the child's birth certificate or an adoption/guardianship order. If someone other than the parent or legal guardian will bring the child, that's when Form 300-01F is needed - handle the paperwork in advance according to the facility's visiting instructions.
Note: Nudity of any kind is not permitted during visits. Children must be fully clothed at all times for their safety.
