Note: If you pay at the cashier’s window in person, they accept cash, money order, or cashier’s check. If you’re mailing your request, stick with a cashier’s check or money order - no personal checks, debit, or credit cards.

ID and Notarization

  • Two forms of government-issued identification
  • Both IDs must be unexpired

Your clearance/pardon form must be notarized - no exceptions. When mailing your request, include a copy of your driver's license or ID card plus a second form of identification, along with the completed notarized form.

  4. Send the original form and payment - the original form and payment must be sent by courier service or through the U.S. mail to the Sheriff’s Department address shown on the form.
