CR-2152 is Tennessee Department of Correction's visitation application - the form you need to complete before you can be added to an inmate's approved visitor list. TDOC's policy recognizes that visits support rehabilitation and healthy family involvement, but everyone still goes through approval first. You don't have to track down the paperwork yourself. TDOC requires facilities to provide inmates with blank CR-2152 forms and written instructions explaining how visitors should complete and return the form with a current photograph. Note: You won’t be admitted for visitation until your CR-2152 is approved. The only exception is for immediate family of newly committed inmates, and that temporary approval can only last up to 60 days from the intake date.

The photo requirement trips up a lot of applicants. CR-2152 requires a recent photograph attached in the lower left-hand corner, and the form warns that applications won't be processed without it. Make sure the photo is current and that you're sending an actual photo - not a photocopy. If you're unsure whether what you have counts, don't risk it. Send a clear, recent photo and attach it exactly where the form specifies.

Once DJRC receives your CR-2152, expect a decision within 30 days. That's TDOC's stated timeline for approving or denying applications. It's not a guarantee you'll hear back on exactly day 30, but it's a solid planning window - especially if you're coordinating travel or time off work.

If your loved one was recently committed, immediate family may be allowed to visit before full approval comes through - but only temporarily. The facility can add immediate family names to the approved list while the application is pending. This temporary approval can't extend beyond 60 days from the inmate's intake date. After that, standard approval rules apply.
