Getting Approved to Visit at Debra K. Johnson Rehab Center: How to Complete and Submit the CR-2152

You won't be admitted to visit someone at Debra K. Johnson Rehabilitation Center until your CR-2152 visitation application is approved. Complete the form, attach a current photograph, and follow the written instructions that come with it. One exception: immediate family of newly committed inmates may visit before full approval. The facility can add immediate family to the approved visitor list temporarily - up to 60 days from the inmate's intake date - while your application is being processed.

The CR-2152 is a TDOC visitation application form. Inmates receive blank copies along with written instructions for prospective visitors. When you fill yours out, include a current photograph - your paperwork isn't complete without it.

Don't skip the photo. The CR-2152 instructions require a current photograph with your application. Missing it will slow things down.

After completing the CR-2152 and attaching your photo, return it to the Deputy Superintendent/Associate Warden in charge of visitation. The form's written instructions explain exactly how the facility wants it routed - follow those directions.

If you need to confirm where your completed application should go, contact Debra K. Johnson Rehabilitation Center directly: 3881 Stewarts Lane, Nashville, TN 37243-0468, phone (615) 741-1255.

Once the facility receives your application, expect a decision within 30 days. Keep this timeline in mind if you're planning travel or coordinating a first visit.

This 30-day window is separate from the temporary allowance for immediate family of newly committed inmates. If your loved one was just taken into TDOC custody and you're immediate family, the facility may add you to the approved visitor list while your application processes - but only for up to 60 days from the intake date. Outside that situation, you'll need to wait for approval before visiting.

  1. Complete the CR-2152 and follow the written instructions - Blank copies are furnished to inmates for prospective visitors to fill out.
  2. Include a self-addressed stamped envelope with your application - New applicants who include one can receive a visitor’s handbook.
  3. Return the application as directed (with your current photo) - The handbook request goes along with your application packet.

Troubleshooting

  • Fill out every part of the CR-2152 before you send it back.
  • Include a current photograph with the application (missing photos are a common reason packets get delayed).
  • Send or route the completed form to the Deputy Superintendent/Associate Warden in charge of visitation, as the instructions direct.
  • Get it returned within the timeframe stated in the instructions (the form is furnished with written directions for returning it within 30 days).

Security screening: Expect multiple checkpoints, including a body scanner. Bring only your photo ID and car keys or key fob into the visitation area. Leave everything else - phones, smart watches, wallets - locked in your vehicle.

If it's been a while since your last visit, don't assume an older application is still valid. Complete a fresh CR-2152 with a current photograph and submit it according to the instructions - especially if you want to avoid delays at the door.
