How to Send Mail to Someone at Debra K. Johnson Rehab Center (DJRC)

As of November 3, 2025, all mail for inmates at DJRC must go to a centralized processing address: P.O. BOX 247, Phoenix, MD 21131. Send your letter to the prison's street address instead and it won't be processed correctly - which means delays.

1. Write the inmate’s committed (legal) name - Use the name TDOC has on file for them.
2. Add the TDOC ID number - Put their TDOC identification number on the address so staff can match the mail to the right person.

DJRC's street address is 3881 Stewarts Lane, Nashville, TN 37243-0468, and you can reach the facility by phone at (615) 741-1255. Use these for questions or general contact - but not for mailing letters. Inmate mail still needs to go to the Phoenix, MD P.O. box.

• ✓ Put your full name in the return-address area.
• ✓ Put your complete return mailing address on the envelope (mail without a return name and address doesn’t meet TDOC’s requirements).

Common Mistakes

• ✓ Leaving off the inmate’s TDOC ID number.
• ✓ Mailing the letter to DJRC’s Nashville street address instead of P.O. BOX 247 in Phoenix, MD.
• ✓ Forgetting to include your return name and return address on the envelope.
