You must be on the adult in custody's (AIC's) approved visitor list before scheduling an in-person visit at Deer Ridge. Even if an online scheduling system lets you pick a time, the institution will turn you away if you aren't approved. Before you book a hotel, request time off work, or start driving - confirm your status. The easiest way? Ask your incarcerated loved one to verify that your visiting status is current.

Step1 Fill Cd50

  • Complete every required field on the CD-50 Visiting Application - incomplete applications will be returned.
  • Submit only one CD-50 per visitor.

Once you've filled out the CD-50, submit it to the Visitor Services Unit by email. Contact Oregon DOC directly for the current email address.

Prefer fax? Send your completed CD-50 to Visiting Services at (503) 373-1173. Double-check that pages are clear and complete - light or cut-off pages can delay processing.

Submitting your application doesn't mean you're approved. The facility still needs to review it - so don't assume you're good to go just because you hit send or because the online scheduling system lets you pick a date. Before you schedule a visit and definitely before you make the trip, confirm your approval. Ask your incarcerated loved one to check whether you're on their approved visitor list. Otherwise, you might show up at Deer Ridge only to be turned away at the door.

Denied? You can request an administrative review. Either the AIC or the prospective visitor can file by completing form CD-1594 and submitting it to Visiting Services.

  1. Get the CD-1594 form and fill it out completely - you can request an administrative review using CD-1594 if your visiting application was denied.
  2. Watch the 60-day clock - Visiting Services must receive your CD-1594 within 60 days of the date on the denial letter.
  3. Send it to Visiting Services - submit your CD-1594 to the Visiting Services Unit using the same contact options used for visitor paperwork (mailing to Visiting Services Unit, 3723 Fairview Industrial Drive SE, Suite 200, Salem, OR 97302-4975, or faxing to (503) 373-1173).

Note: The Director's designee makes the final call on administrative reviews - there's no further appeal after that.

Appeal Checklist Docs

  • A completed CD-1594 (Request for Administrative Review)
  • A copy of the CD-50 you originally submitted
  • A copy of your denial letter (so you can show the date and stay within the 60-day “received by” deadline)
  • A copy of your ID
  • A short written explanation of why you’re requesting review (for example, if the denial was based on incorrect information or if something relevant has changed)
