Bringing Children to Visit at Potomac Highlands: Forms, IDs, and Who Can Accompany Them
Bringing a child to visit at Potomac Highlands takes extra prep. Missing documents and wrong adult accompaniment are the biggest reasons families get turned away, so getting everything ready before you go makes all the difference.
Anyone under 18 visiting Potomac Highlands must present the child's birth certificate. Have it with you when you arrive so check-in doesn't stall at the front desk.
Minors cannot visit on their own. A parent or legal guardian must accompany the child. If the parent or legal guardian isn't coming, the child needs to be with an authorized adult visitor instead.
When a parent or legal guardian won't be present, Potomac Highlands requires extra paperwork. The juvenile must have a signed, notarized Juvenile Visitation form. Without it, the facility can refuse the visit, even if the child has their birth certificate and an adult is there.
Note: The facility's notaries cannot notarize the Juvenile Visitation form. You'll need to get it notarized outside the facility before your visit.
You can get the Juvenile Visitation form from the facility or online through dcr.wv.gov. If you're planning a visit soon, I recommend grabbing the form as early as possible so you have time to fill it out and get it notarized.
Juvenile visitation requests can involve a background check. The guidance included with the Juvenile Services Visiting Request says to contact the facility where the offender is located to confirm background check approval and schedule visits. Completed Juvenile Services visiting requests should be mailed to: WV Division of Corrections & Rehabilitation, Bureau of Juvenile Services, Attn: Visitors Request, 1409 Greenbrier Street, Charleston, WV 25311.
Note: The Superintendent has the authority to deny a juvenile visit even if the background check has been approved.

Documents to Bring on Visit Day

- ✓ A signed, notarized Juvenile Visitation form (required if the child is not accompanied by a parent or legal guardian)
Bring the original birth certificate and keep it handy during check-in. If you're using the Juvenile Visitation form (available from the facility or through dcr.wv.gov), keep the notarized original with your visit paperwork too. You don't want to be scrambling at the window.
