How to Set Up Attorney Visits at Arkansas DOC Facilities (Form Guide)
Getting approved for attorney phone calls or visits with someone in the Arkansas Department of Corrections starts with one form: the Attorney Visitation/Telephone Request. Here's what the form asks for and where to send it.
Fill out the form completely and legibly. If a section doesn't apply to you, leave it blank - but make sure everything else is filled in clearly. Incomplete forms slow things down.
At the top of the form, you'll select what type of access you need: PHONE, VISITATION, VIDEO VISITATION, or ALL (which includes video). Pick the one that matches your situation - this determines which access list you'll be added to.
By submitting this form, you're agreeing to a criminal background check. This consent is required - without it, your request won't be processed.
Note: The form also includes consent that all persons, property, and vehicles entering Department property are subject to search.
Identity and Attorney Info
- ✓ Provide your State Bar number.
- ✓ Print your full name exactly as it appears on your state-issued photo ID or driver’s license.
- ✓ List your state-issued photo ID or valid driver’s license number and the state of issuance.
- ✓ Provide your date of birth.
- ✓ Provide the last four digits of your Social Security number.
Send the completed form to the inmate's assigned unit, addressed to the Unit Visitation Clerk. That "attention to" line matters - it ensures the form reaches the right office.
