Getting on an Inmate's Visiting List: What to Expect from the Background Check

Trying to get approved to visit someone in federal custody? The first hurdle is paperwork. Here's what the BP-A0629 background questionnaire covers, how it's used, and what can cause delays or denials.

The BP-A0629 questionnaire exists because the Bureau of Prisons has legal authority under Title 18 U.S.C. § 4042 to request background information on proposed visitors. The practical reason is simple: staff need to check whether adding you to an inmate's visiting list would create a management problem for the institution or cause other adverse effects. As part of that review, institution officials can send an inquiry to a law enforcement or crime information agency.

Note: Your information can be checked with a law enforcement or crime information agency as part of the decision about whether you are approved to visit.

You'll need to complete the BP-A0629 questionnaire and return it to the institution address listed on the form. Filling it out is technically voluntary. But if you don't provide the requested information, processing gets suspended and you won't receive any further consideration.

  • A completed BP-A0629 questionnaire and release form, returned to the institution address listed on the form
  • If you are entering or corresponding as the representative of a licensed attorney: the BP-A0243, which is organized into a Questionnaire, a Certification you sign, and an Attorney’s Statement the licensed attorney signs
  • The information required to complete a BP-A0660 criminal history check authorization (this check must be completed before a person is allowed to enter or serve at any Bureau facility)

Note: A criminal history check (BP-A0660) has to be completed before a person is allowed to enter or serve at any Bureau facility. Refusing to provide the necessary information can lead to denial of entry and denial of volunteer or contract status.

Once you submit the BP-A0629, the institution can verify everything you provided. The form specifically warns that officials may send an inquiry to a law enforcement or crime information agency to determine whether putting you on an inmate's visiting list could create a management problem or other adverse effects.

The Bureau cites Title 18 U.S.C. § 4042 on the BP-A0629 as its legal authority to request background information on proposed visitors. That's the statute they point to when asking for this information as part of the approval process.

Be accurate when you fill out the BP-A0629. The form warns that making false statements can trigger criminal penalties under 18 U.S.C. § 1001, including a fine of up to $250,000, imprisonment for up to five years, or both. This isn't a formality. Answer honestly.

  1. Complete the BP-A0629: Fill out the questionnaire and release form, then return it to the institution address listed on the form so you can be considered for visiting privileges.
  2. Expect a background review before approval: Part of the process may include a criminal history check (BP-A0660). The authorization form explains that the check must be completed before a person is allowed to enter or serve at any Bureau facility.
  3. Avoid delays by providing the required information: The BP-A0629 explains that you are not required to supply the information requested, but if you do not furnish it, processing may be suspended and you may receive no further consideration. Refusal to provide all necessary information for the BP-A0660 may also result in denial of entry and denial of volunteer or contract status.

Note: If you refuse to provide the information needed for the BP-A0660 criminal history check, you can be denied entry into a Bureau facility and denied volunteer or contract status.
