Anyone 12 or older needs to complete their own Florida DOC visitation application. You can't be added under someone else's paperwork - each visitor submits their own form.

Before submitting, read through the visitor policies attached to the application. These are the rules you're agreeing to follow, and they're what staff will use when deciding whether your visit gets approved.

Send one application only. Submitting duplicates because you're worried it didn't arrive just creates confusion and slows everything down. Focus on getting one complete, correct form in.

Common Denial Mistakes

  • Fill in every required field - leaving blanks can cause your application to be denied.
  • If a question truly doesn’t apply to you, write “NA” (not applicable) instead of leaving it empty.

Don’t skip the signature: If you don’t sign the application in the space provided, it will not be processed.

Wait for approval: Do not go to visit until the inmate notifies you that your visitation application is approved.
