ADOC uses a single release form for inmate medical records: the Inmate Authorization for Release of Health Records. Once it’s filled out and signed, email the completed form to ohs.medicalrecords@doc.alabama.gov for review by ADOC’s Office of Health Services.

Read the form carefully before signing. Certain categories of information won't be released without expressed consent. This includes records related to HIV, sexually transmitted diseases, psychiatric/mental health, and drug or alcohol treatment. If you're requesting any of these - or aren't sure what the file contains - complete the sensitive-records consent section exactly as the form requires. Skip this step, and those portions may be excluded from what you receive.

Reminder: If the sensitive-records consent section isn’t explicitly completed, ADOC may release general records but hold back HIV/STD, mental health/psychiatric, and drug/alcohol treatment information.

The authorization stays valid for one year from the date it's signed. Need to cancel it? That has to be done in writing. Alabama law also allows a reasonable fee for producing records, so be prepared for a possible charge.

How to Request Medical Records for an Inmate at Fountain Correctional Center

  8. Follow up if you don’t get confirmation - If you haven’t heard back after a reasonable amount of time, reply to your sent email thread with a brief request for status.
