How to Apply for Visiting at Four Mile Correctional Center: From Form to Approval
To visit someone at Four Mile Correctional Center (FMCC), you'll need to complete the Visitor Application (AR 300-01A). Download it from the facility's Visiting Applications page, then email the completed form to DOC_ecc-visiting@state.co.us. Double-check that you're submitting the form for the correct facility before hitting send.
Required Docs ID Address
- ✓ A copy of a current, valid, unexpired government-issued photo ID
- ✓ Double-check that your ID is not expired (expired IDs don’t meet the requirement)
- ✓ Make sure the address on your photo ID matches the address you write on your visiting application (a mismatch can lead to denial)
Each adult needs their own application - FMCC only allows one adult name per form. Once submitted, the facility's Administrative Head reviews and approves, denies, or deactivates visitors on the visiting list.
After you're approved, Colorado DOC runs automatic background checks every year to confirm you don't have active warrants, detainers, or unresolved criminal cases. One thing to watch out for: if you go a full year without visiting, you'll be automatically deactivated from the visiting list. You'd need to reapply to get back on.
Video visits at FMCC require the same visitor application as in-person visits. Get approved first, then set up your account through Securus Video Connect to schedule video sessions.
Even with approval, visits aren't unlimited. The facility can restrict the number of visitors and visit length based on schedule, space, and staffing. Submit your AR 300-01A as early as possible so you're ready when time slots open up.
Common Rejection Reasons
- ✓ Your photo ID copy is missing, expired, or not government-issued
- ✓ The address on your photo ID doesn’t match the address on your application
- ✓ The application is incomplete (blank fields, missing signature, or missing required info)
- ✓ You didn’t use the correct form (Visitor Application AR 300-01A)
- ✓ You submitted it the wrong way instead of emailing DOC_ecc-visiting@state.co.us
Note: Fixing paperwork issues doesn't guarantee approval - the Administrative Head makes the final call.
- Get the Visitor Application (AR 300-01A) - Use the Visiting Applications page so you’re filling out the correct form.
- Fill out the application carefully - One adult name is permitted per application, so each adult should complete their own.
- Attach a copy of your unexpired government photo ID - The address on your ID must match the address you put on the application.
- Submit your application to the facility - Email it to DOC_ecc-visiting@state.co.us (and make sure it’s being returned to the facility where the offender is housed).
- Wait for the decision - The Administrative Head is the authority who approves, denies, or inactivates visitors.
- If you want video visits, set them up after approval - Once you’re approved as a visitor, you can create an account and schedule video sessions through Securus Video Connect.
- Stay aware of ongoing checks and status changes - Active visitors are automatically background-checked yearly for warrants, detainers, or unresolved cases.
Tip: Haven't visited in a year? You'll be automatically deactivated and need to reapply. Build in extra time so this doesn't delay your next visit.
