How to Fill Out Oklahoma's Visitor Request Form (so it isn't denied)

Oklahoma requires visitors to submit the official Visitor Application Form (also called the Visitor Request Form, DOC 030118B) to get on an inmate's approved visitors list. This form goes beyond basic contact info. By signing it, you're agreeing to several conditions Oklahoma DOC uses to decide whether to approve you, including permission for a criminal background check and consent to the facility's search rules when you step onto prison grounds.
Warning: By signing the Visitor Request Form, you consent to pat-down searches and vehicle searches when you enter prison property (and possibly more intrusive searches based on staff suspicions). Your signature also authorizes Oklahoma DOC to run a criminal background check.
ID Checklist
