How to Request Medical Accommodations for Visiting at GBCI

If you need a medical device or have a physical condition that affects screening at Green Bay Correctional Institution (GBCI), get your accommodation approved before you arrive. GBCI uses one specific form—DOC-2424—and it must be verified and approved in advance.

How to Request Medical Accommodations for Visiting at GBCI

You'll need a medical accommodation if your physical condition or a medical device prevents you from clearing the metal detector. GBCI doesn't handle these exemptions on the spot. File DOC-2424 with the institution and get it approved ahead of time - otherwise you risk being turned away at the door.

DOC-2424 is a shared form. You fill out the visitor portion first, then take it to your healthcare provider so they can complete the medical section.

Note: GBCI requires DOC-2424 for medical exemptions. Both you and your medical professional must complete it before you visit.

  1. Have the completed DOC-2424 faxed - Once you and your medical professional have filled it out, it must be faxed to (920) 432-5388.
  2. Wait for Security Department verification - GBCI’s Security Department verifies the request as part of the approval process.
  3. Watch for your written decision - If your accommodation is approved, you’ll be notified in writing.

Note: GBCI requires the DOC-2424 to be faxed from your physician’s office to (920) 432-5388 so it can be verified for approval.

Once approved, GBCI will notify you in writing. Keep that approval on file before you arrive - the Security Department verifies accommodations ahead of time, not at the Visiting Center on the day of your visit.

  • Nitroglycerin
  • Inhalers
  • Epinephrine (epi-) pens
  1. Tell the Visiting Center Officer you have medication - Let staff know right away when you arrive.
  2. Turn the medication in to the Visits Officer - Even allowed medications have to be handed over during check-in.
  3. Use medication only at the Visits Officer’s desk - It may only be used at that desk, and only in the presence of security staff (Officers/Sergeants).

Practical Tips for Physician Offices

  • Fax DOC-2424 directly from the physician’s office to (920) 432-5388.
  • Ask the office to keep confirmation that the fax was successfully sent.
  • Make sure the visitor’s name is clear on the fax so it can be matched to the request.
