How to Request Medical Accommodations for Visiting at GBCI (DOC-2424)
If you need a medical accommodation to get through the metal detector at Green Bay Correctional Institution (GBCI), you have to get it approved before you show up. Here's how the DOC-2424 process works and what to expect at the door.
Any medical exemption for the metal detector at GBCI must be filed and approved before your visit. The facility only accepts the DOC-2424 form for this. Don't plan on bringing a doctor's note or other documents on the day of your visit. It has to be the DOC-2424, submitted ahead of time.
On the day of your visit, don't show up too early. Arrive no more than 15 minutes before visiting hours start. Once you're in the lobby, you may need to fill out a DOC-176 (Request to Visit Offender) as part of check-in.
You can't complete the DOC-2424 on your own. You fill out your portion, and your physician handles the rest. Both parts need to be completed before submitting.
- ✓ Fill out the visitor portion of DOC-2424
- ✓ Ask your physician to complete the medical professional portion of DOC-2424
- ✓ Make sure the form reflects both your information and your physician’s input before it is sent
- Have your physician’s office fax the completed DOC-2424 to (920) 432-5388.
- Wait for Security Department review so the request can be verified and approved (or not approved) before you attempt to visit.
Don't assume your accommodation is in place until you've received written approval. GBCI requires these medical exemptions to be filed and approved before you visit. If your DOC-2424 is approved, you'll get a written notice. Wait for that before making the trip.
Even with an accommodation on file, expect staff to verify that your medical exemption was approved before letting you through. Approval comes in writing, and the key here is that everything must be settled before you arrive. You won't be able to negotiate it at the front entrance.
Critical: You will be denied entry to the visiting room if you can't clear the metal detector within three attempts.
Keep your timing tight on visit day, even with an approved accommodation. Arrive no more than 15 minutes before visiting hours start. Staff may ask you to complete a DOC-176 when you check in at the lobby.
