How to Complete the CR-2152 to Visit Someone at Hardeman County Corrections

If you want to visit someone at Hardeman County Corrections, the CR-2152 is the first hurdle. Here's how to get the form, fill it out correctly, attach the right photo, and submit it so it gets approved without avoidable delays.

The CR-2152 is Tennessee's visitation application form. Blank copies are provided to inmates, so the person you want to visit can get one inside and help you start the process.

You need to complete the CR-2152 and return it with a current photo before you'll be considered for visiting. Until your application is approved, you won't be admitted for visitation. There is one limited exception for immediate family of newly committed inmates, which I'll cover below.

Start by getting a blank CR-2152 from the inmate you plan to visit. Blank copies are furnished to all inmates. You can also get a copy of the visitor handbook by including a self-addressed stamped envelope with your application. The handbook is posted in visitor areas too, so you may see it when you arrive at the facility.

Fill out every section on the CR-2152, then attach your photo before sending it back. The form requires a recent photograph in the lower left-hand corner. Applications without that photo will not be processed. Once it's complete, return it to the visitation authority as directed (this goes through the Deputy Superintendent or Associate Warden in charge of visitation).

  • If you are applying for a child visitor age 6 or older, attach a photograph to the child’s visitor application. It cannot be a photocopy.
  • Plan to update the child’s photo at ages 10, 12, 14, 16, and 18.

Address rule: Do not use a P.O. Box as your address on the CR-2152. The form does not allow it.

Once your CR-2152 is complete with your current photo attached, send it back as instructed so it reaches the Deputy Superintendent or Associate Warden in charge of visitation. The form should be completed and returned within 30 days.

After the facility receives your application, expect a decision within 30 days. If you're planning travel or coordinating multiple visitors, use that 30-day window as your baseline.

There is one situation where a visit can happen before the CR-2152 is approved. Immediate family of newly committed inmates may be admitted temporarily while the application is still pending. The Warden can add names to the approved list until the application is received and approved.

For the regular approved visiting list, all immediate family members who apply can be approved, plus up to eight additional adults, upon receipt of the CR-2152. Everyone still needs an approved application on file before being admitted (outside of the newly committed immediate-family exception).

  • Submitting a photocopy instead of an actual photograph for a child age 6 or older.
  • Forgetting to include the required child photo at all.
  • Using a P.O. Box as your address on the CR-2152.

Anything missing from your application can slow the timeline. The facility aims to approve or deny applications within 30 days of receipt, so turning in a complete CR-2152 with the required current photo gives you the best shot at staying inside that window.
