Visiting someone in custody is easier when you know which forms to use. First, get approved as a visitor. Then submit a request for the specific visit you want.

Before you can visit, you need to become an approved visitor. Start with the Visitation Request Form. Fill it out and submit it to the facility where your loved one is incarcerated. Approval and scheduling are separate steps. If you haven't been approved yet, your visit request won't move forward. Make this your first task. If the facility asks for additional verification, follow their instructions closely and submit exactly what they request.

Once approved, use the facility's Request a Visit form to ask for a specific date and time. This guide doesn't cover visiting hours, ID requirements, dress code, or how far in advance you need to schedule. Confirm those details directly with the facility before you go so you don't get turned away at check-in.

Steps to Follow

  • Fill out the Visitation Request Form and submit the completed form to the facility where the person is incarcerated.
  • Submit a visit request through the facility’s Request a Visit form.
  1. Locate your scheduled visitation: Use the facility’s visit tools to find the appointment you already scheduled.
  2. Add another visitor (if needed): Use the “Add Visitor” link to find your scheduled visit, then add additional visitors to that visit.
  3. Cancel a visit (if plans change): Use the “Cancel my visit” link to find your scheduled visitation, then cancel it.

Note: Approval, scheduling, adding visitors, and cancellations all use specific facility forms. If you can't find the form you need, contact the facility directly to confirm the current process.

Before submitting anything, confirm you're using the current Visitation Request Form and the current Request a Visit form. Forms and instructions change, and using an outdated version can delay your approval or scheduling. Also verify how the facility wants the Visitation Request Form delivered. The requirement is to submit it to the facility where your loved one is incarcerated, but the exact method (in person, mail, or online) may vary.
