How to Visit Facility

You'll request visits through the GDC visitation form process. First, though, you need to become an approved visitor. That means filling out the Visitation Request Form and submitting it to the facility where your loved one is housed. Once your form is on file and you're approved, you can start scheduling visits through the same system.

1. Get the Visitation Request Form - This is the form used for the GDC visit request process.
2. Fill it out completely - Approval starts with a complete Visitation Request Form.
3. Submit the completed form to the facility where the person is housed - The form must be sent to the facility holding your loved one.
4. Wait for approval before planning the trip - Visits are tied to the approved-visitor process and the scheduled-visit request.

• ✓ If you already have a scheduled visit and need to add someone, use the “Add Visitor” option to find your scheduled visitation and add additional visitors.
• ✓ If you need to cancel a scheduled visit, use “Cancel my visit” to locate the appointment and cancel it.

Before making plans around a visit, double-check two things: that you're using the correct Visitation Request Form, and that the facility actually received your completed form. It's also smart to confirm any facility-specific procedures that could affect your trip, like the current visiting schedule or what identification they require at check-in.
