Bringing a Child to Visit at Jess Dunn: Documents, Signatures, and What Changes at 18

If you're bringing someone under 18 to visit, you'll need to prove you have the legal right to bring them. Oklahoma DOC requires guardianship documentation for minors, such as a birth certificate, court papers, or adoption papers. The visitor request form also requires a parent or legal guardian's signature for anyone under 18. No signature, no approval. The request will be denied.

Every visitor needs to submit a Visitor Application Form. If you're using a paper application, ODOC still accepts it at headquarters. Mail it to the Visitation Unit at P.O. Box 11400, Oklahoma City, OK 73136-0400.

Documents to Bring

  • The child’s birth certificate
  • Court paperwork showing legal guardianship (if applicable)
  • Adoption papers (if applicable)

Required: For visitors under 18, the parent or legal guardian's signature must appear on the visitor request form. Without it, the request will be denied.

Once the child turns 18, they're no longer covered under the minor visitor paperwork. They'll need to apply in their own name as an adult visitor. That means passing a background check before visiting, which can take up to eight weeks. They'll also need photo identification. Acceptable IDs include a state driver's license, or a state, federal, military, or school ID.

  1. Submit the Visitor Application Form. This is required for all visitors.
  2. Allow time for the background check (18+). For visitors 18 and older, approval can take up to eight weeks.
  3. Watch for confirmation through the incarcerated person. The inmate will be notified when the background check is complete, and they will also be notified of the scheduled date and time of visitation.

Guardian Checklist Dayof

  • Documentation that verifies guardianship for the minor (birth certificate or court or adoption papers)
  • Your photo ID if you are 18 or older (state driver’s license, or a state, federal, military, or school ID)
  • A completed Visitor Application Form (paper forms are accepted at ODOC headquarters and must be addressed to the Visitation Unit at P.O. Box 11400, Oklahoma City, OK 73136-0400)

Reminder: Any visitor under 18 needs a parent or legal guardian's signature on the visitor request form. Without it, the request will be denied.
