Bringing a Child to Visit at Lawton Community Corrections Center: Paperwork Checklist

Everyone visiting Lawton Community Corrections Center needs approval through the Oklahoma Department of Corrections (ODOC). That means submitting a Visitor Application Form first - no application on file, no visit.

Any visitor under 18 needs a parent or legal guardian to sign the Visitor Request Form. ODOC requires the adult's printed name and signature - skip this, and the request gets denied. Before you submit anything, double-check that signature section.

Documents Prove Guardianship

  • Birth certificate (to show parentage)
  • Court paperwork showing legal guardianship (if you’re the guardian)
  • Adoption papers (if applicable)

Bringing a child? You'll need a government-issued photo ID. ODOC requires one from every adult visitor.

Checklist Day of Visit

  • Visitor Application Form submitted for each visitor before trying to visit
  • Visitor Request Form for the minor, completed with the parent/legal guardian’s printed name and signature (missing signature = denied)
  • Your original government-issued photo ID (if you’re 18+)
  • A copy of the front and back of your driver’s license or state-issued ID to submit with the request
  • Proof of guardianship/parentage for the child (birth certificate, court guardianship paperwork, and/or adoption papers)
  • If you’re using paper forms: mail the visitation application to ODOC headquarters addressed to the Visitation Unit (do not send it back to the inmate)
  • Mailing address for paper applications: Oklahoma Department of Corrections, Attn: Visitation Unit, P.O. Box 11400, Oklahoma City, OK 73136-0400

Timing tip: Background checks for visitors 18 and older can take up to eight weeks. The inmate gets notified once approval comes through, so start early if you're planning a first visit.

When the child turns 18, they're no longer covered as a minor visitor. They'll need to submit their own visiting application to continue visits.
