How to Submit Your Visitation Application to Madison Correctional Institution (Madison CI)

To submit by email, send your completed application to VisitappMadisonCI@fdc.myflorida.com. Double-check that the form is complete and signed before hitting send - this avoids delays from back-and-forth.
You can also mail your application to the institution. If you go this route, keep a copy for your records.
Stick to one form per person. Sending multiple applications for the same visitor creates confusion and slows things down. Submit one complete, signed application and wait for the decision.
After you submit, wait for approval before making the trip. The inmate will notify you once your application is approved - don't show up at Madison CI until you hear from them.
Do not go to the institution yet: “PLEASE DO NOT VISIT UNTIL THE INMATE NOTIFIES YOU YOUR APPLICATION IS APPROVED.”
