Bringing Your Child to Visit at Mass Treatment Center: ID Requirements and Consent Forms

Minors under 18 don't need pre-approval to visit at Mass Treatment Center. The key requirement is who brings them: your child must arrive with a parent or legal guardian who has physical custody, and that adult must already be on the inmate's approved visiting list.

You'll need a Minor Consent Form if someone other than the parent or legal guardian with physical custody is bringing the child. That adult must submit the completed form and get the Superintendent's approval before the visit. Planning to send your child with another trusted adult? 1. Get and fill out the Minor Consent Form - the form must be completed by the adult who will bring the child.
2. Submit the completed form - it must be turned in to the facility as required.
3. Wait for Superintendent approval - approval must be obtained before the minor can visit with a non-custodial adult.
