Visitation Application Checklist for Mayo C.I. Annex (Mayo CI)
Important: Do not go to Mayo C.I. Annex for a visit until the inmate tells you your visitation application has been approved.
Submit your completed application to Mayo C.I. Annex by email or U.S. Mail - pick one method. Make sure the form is fully filled out before sending.
Submitting by email? Send your application to VisitappMayoCI@fdc.myflorida.com. Double-check the address - typos mean bounced emails.
Everyone who wants to visit must submit their own application - one form per person. If multiple family members are applying, coordinate ahead of time to avoid duplicate submissions.
Anyone 12 or older must complete their own visitation application to visit Mayo C.I. Annex.
Don't leave any questions blank. The facility can deny incomplete applications. If something doesn't apply to you, write "NA" instead of skipping it.
Stop before you send it: Make sure you sign the form where it asks for your signature - unsigned applications won’t be processed.
Be truthful and double-check your details before submitting. False or misleading information will get your application denied.
