Your Visitor Application must go to the Visitation Unit - not to the person you're trying to visit. Even if you want help filling it out, don't send it to the inmate. ODOC accepts paper forms at their headquarters in Oklahoma City. Address your envelope exactly like this: Oklahoma Department of Corrections Attn: Visitation Unit P.O. Box 11400 Oklahoma City, OK 73136-0400.

Note: Do not return the Visitor Request Form to the inmate. If it’s returned to the inmate, it will be rejected.

What-to-include

  • Copy of the front and back of your driver’s license or state-issued ID (send this with the Visitor Request Form)
  • Photo ID for anyone age 18 or older to carry when you actually come to visit
  • If the visitor is under 18: parent or legal guardian signature on the Visitor Request Form (without it, the request will be denied)
  • If you’re bringing minors: documentation to verify guardianship (for example, birth certificates or court/adoption papers)

Leaving things blank or trying to "clean up" an answer is one of the fastest ways to get denied. ODOC's rule is simple: falsify or purposely omit information, and you lose visitation privileges. You'll also have to wait 90 days before reapplying. Take a few extra minutes to fill every space and double-check your answers before mailing.

Common rejection reason: If the form is returned to the inmate, it will be rejected.

  1. Expect a background check if you’re 18 or older - ODOC runs background checks for visitors age 18+ as part of the application.
  2. Plan for processing time - approval can take up to eight weeks.
  3. Watch for the final word through the inmate - the inmate will be notified when processing is complete and will also notify you of the scheduled date and time of visitation.
