When placing your order, format the shipping label to include the incarcerated person's committed (or legally changed) name, their institutional number, the facility name, and the facility post box number. It should look something like: "[Name] [Institutional Number] / Omaha Correctional Center / [Facility Post Box Number] / [City, State ZIP]."
