The CD-50 visiting application is the form Oregon DOC uses to decide whether someone can be added as an approved visitor. Either the adult in custody (AIC) or the person who wants to visit can submit it - but submitting isn't the same as being approved. The AIC has the right to refuse visiting requests, and they're responsible for monitoring and maintaining their own visiting list.

Information Needed

  • Your full legal name
  • Current address and phone number
  • Date of birth
  • Driver’s license number (and issuing state)
  • Any other names you’ve used (aliases/maiden names)
  • Your relationship to the AIC (so it’s clear who is being added to their visiting list)
  • Any identifying number requested on the form (such as a SID, if applicable)
