What to Do If Your Visiting Application Is Denied (how to request administrative review)
Denied? Act fast. Your request for administrative review must be received by Visiting Services within 60 days of the date on your denial letter. That's "received by" - not postmarked. Don't wait until the last minute to mail it. A late arrival means you lose your chance at review.
You'll need form CD 1594, titled "Request for Administrative Review." Fill it out completely: your name and address, the AIC's name, SID, institution, and your relationship. The most important part is your written explanation - use that space to clearly connect the denial reason to the facts and documents you're submitting.
Don’t submit it “bare.” CD 1594 instructs you to attach all documents you want considered for the administrative review.
Supporting Documents
- ✓ A copy of the denial letter (so the reviewer can see the reason given)
- ✓ Corrected or updated information that addresses the reason for denial
- ✓ Proof that a prior mistake or incorrect information affected the decision
- ✓ Official records showing circumstances have changed (if that’s what you’re relying on)
- ✓ Any other paperwork you want the reviewer to consider - include it now, not later
Label your attachments clearly ("Attachment A," "Attachment B") and reference them directly in your explanation. Writing "see Attachment B" right where it supports your point helps ensure nothing gets overlooked.
Mail your completed CD 1594 to: Visitor Services, 3723 Fairview Industrial Drive SE STE 200, Salem, OR 97302-4975. Before you send it, double-check the form for any submission instructions and make sure everything you want considered is included in the same packet.
This is your one shot. The Director's designee makes the final decision - there's no further appeal after that. Take the time to write a clear explanation and include every supporting document the first time around.
