Maryland Restricted Information (MDJ-008): When to File the Notice Regarding Restricted Information
  1. Look for restricted information before you file - If your CC-DR-004, CC-DR-050, or CC-DR-095 includes information that’s confidential by statute, rule, or court order, you’ll need to flag it.
  2. Check the “Restricted Information” box on the main form - Each of these forms instructs you to check the box when your submission contains restricted information.
  3. Attach MDJ-008 to the same submission - File the Notice Regarding Restricted Information (MDJ-008) with the form you’re submitting, as the form instructions require.
