How to Request a Booking Summary from RADF (form, fee, ID, and submission)

The Booking Summary Request form is what you'll use to ask the Records Division for a booking summary. Fill it out with the person's identifying details and your own contact information. Staff will use this to process the request and get the results back to you.

There's a $15 processing fee. The form also has a spot for a receipt number, so hang on to any proof of payment you receive.

You need to include a copy of a valid ID with your request. The form specifically instructs applicants to attach one, and skipping this step can delay processing.

Once the form is filled out and your ID copy is ready, mail everything to: P.O. Box 1040, 328 Applestill Rd., El Centro, CA 92243.

Tip: If you have questions about your request, call the Records Division at (442) 265-2125 before you submit.

Would you rather pick up the booking summary in person instead of waiting for it by mail? Call ahead to confirm the current pick-up procedure and what you'll need to bring.

If you ask for additional copies as part of a public records request, extra copying charges may apply. Imperial County’s civil fee chart lists a copying fee of $1.25 per page.
