For in-person form pick-up and drop-off, go to the Rock County Sheriff’s Office Front Office at 200 East U.S. Highway 14, Janesville, WI 53545.

The Front Office is open Monday through Friday, 7:00 a.m. to 4:00 p.m. That's your window to grab forms or turn in completed paperwork.

Looking for Project Lifesaver paperwork? Head to the Rock County Sheriff's Office Community Corrections entrance. A staff member there can hand you a Project Lifesaver application and the Program Contract, Monday through Friday between 7:00 a.m. and 4:00 p.m.

Project Lifesaver has a one-time $25.00 setup fee when a client is enrolled in the program.

On top of that, there's a $10.00 per month fee for battery and equipment maintenance. This monthly cost is billed quarterly to the caretaker listed on the application, so double-check that the caretaker information is accurate on the paperwork you turn in.

Some forms come in more than one format. Ride-along request paperwork, for example, may be available as a fillable PDF, a standard PDF for handwritten responses, or as a paper copy you can pick up at the Sheriff's Office Front Office.

  1. Complete the Ride-Along Request Form - Fill it out fully before you send it in.
  2. Make a copy of your driver’s license - A copy of your driver’s license needs to go with the form.
  3. Email both items to the designated contact - Send the completed form and the driver’s license copy to Sergeant Andy Reed at andrew.reed@co.rock.wi.us.
