It says to send your deposit and completed slip there. But the Commissary and Trust Fund page includes a warning: "Do not send inmate mail to PO Box 60; it MUST be mailed to their unit of assignment." That directly conflicts with the flyer. Both are official sources, and they don't line up. Treat the address as something you need to verify for your specific situation. Call the unit before mailing a money order or cashier's check.
