Already submitted this form for the same offender at another juvenile facility? Don't send it again unless that submission is more than a year old. Contact the facility where the offender is currently housed to find out what they need from you.

1. Make a copy and note your send date - keep a copy of what you mailed (and the date you mailed it) so you can reference it if anything gets delayed.

3. Contact the facility to schedule once you’re cleared - you’re expected to contact the facility where the offender is located for background-check approval and to schedule visits. Even with an approved background check, the Superintendent still has authority to deny a visit.
