Before you try to schedule an in-person visit, make sure you're on the adult in custody's approved visitors list. Oregon DOC lets you schedule through the system even if you're not approved - but the institution will deny the visit if your name isn't on the list when you show up.

Note: Submitting the CD 50 isn’t the same as being approved. The adult in custody can also refuse visiting requests from prospective visitors.

Fill out every line on the CD 50 carefully. Oregon DOC requires all information on the application. Miss something, and they'll send it back instead of processing it - which means more waiting. Take your time and double-check before you send it.

  • Submit one application per visitor (don’t send duplicates for the same person).
  • Make sure all information is filled in - incomplete applications will be returned.

Want to submit by email? Send the CD 50 to DOC.Visitors@doc.oregon.gov (the Visiting Services Unit). It's one of the easiest ways to get your application in without mailing paperwork.

You can also fax your application to (503) 373-1173. If you go this route, confirm the pages went through clearly - missing or unreadable information can slow things down.

To submit by mail, send the CD 50 to: Visiting Services Unit, 3723 Fairview Industrial Dr. Suite 200, Salem, OR 97302. Mailing takes longer, so plan ahead if you're hoping to visit soon.

  2. Don’t rely on the scheduling system alone - even if you’re able to schedule, the visit can still be denied if you aren’t actually approved.
  3. If you’re not approved, check in with the adult in custody - since they can refuse visiting requests, they may be able to tell you whether they’ve declined or if something else is holding it up.
